Request Prescriptions from Heartland Cardiovascular Center

Prescriptions and refills for established patients are routinely issued Monday through Friday between the hours of 9:00am and 4:00pm by calling (815) 740-1900 or by having the pharmacy fax the request to (815) 723-3922. To request a prescription refill online, please fill in the form below. Once submitted, we will respond to the request within 24-48 hours. Please note that refills will not be issued during the evenings, weekends or on holidays as the doctors do not have access to your medical records during those times. Try to anticipate your refill needs to avoid an interruption in your medication therapy.
